It's been pretty smooth since setting up reCaptcha.  As much as people criticize it, reCaptcha is basically the savior of server admins.  None of the other options I tried had any effect.  Blocking bad IP ranges, honey pots, skill-testing questions.  Bots found ways around all of it.

reCaptcha was so effective that for a while I was worried that registrations were simply broken.  Had to create a test account just to make sure it still worked.

On my dead-ass SMF I added questions like "What is 'tree' spelled backwards?" to the register form.

Super fragile if anyone out there decides to program bots specifically for this specific dead-ass SMF, but no bot issues for years since.

I made a change to make stuff better.  Let me know if I made stuff worse instead.

Just a syntax thing to prevent the error log from filling up.

Was actually rescheduled to today, the 30th.  Might see some performance improvements afterwards, though I'm not sure we'd be able to notice it.

Had to repair a couple database tables that had crashed.  Hopefully no ill effects will be felt.
